NON-COVERED CHARGES

A charge is sometimes considered to be non-covered by your insurance company even though it is a necessary part of your visit.

Some examples would be:
refractions
surgical trays
after-hour care
drivers ilcense forms

Driver's license forms are completed as a courtesy on the day you visit. If forms are brought in on a day other than the day of your visit, there is a $10.00 charge.

If your insurance contract states your are responsible for non-covered charges, you will be billed for them.

As a reminder; your vision coverage may be through an insurance company that is different from your medical insurance. You are responsible to know the name of your vision plan and the names of it's providers.

If your physician feels your prescription needs to be checked; he may perform a refraction. This is the part of teh exam spepcific to the determination of whether or not you need corrective lenses or a change in your current prescription. Most medical insurances such as Medicare, Medicare replacements, State Employee BC etc., do not cover this part of the exam. If you do not want this part of the exam done, please let your nurse know when you first get in the exam room. However, keep in mind, you cannot get a current prescription without a refraction.
